The reasons I have heard for the failure of the kamikaze program to be really effective were: a) it wasted expensive equipment--planes and pilots (even green ones need training), and b) it lacked a feedback loop to correct operational mistakes.

Another reason was the U.S. Navy's VT Proximity Fuze, one of the great allied secret weapons of WWII. It's a classic story of American initiative and ingenuity...
