In FO, voice isn't much of an issue, which is another reason it's a poor comparison.

<The last mile problem will be solved when companies can identify the compelling applications and services that will generate revenues and profits. IMO, the current offerings fall way short on both fronts, and represent solutions looking for a problem.>

Are you familiar with ERP, CRM, distance learning or video conferencing? They're there, but companies have to buy point to point frame to run them. The alternative is VPN, but both ways it's too expensive to implement on a wide scale. Look at a company like GENU...50-60% of their gross revenues are circuits, which is pass-through revenue to the ILECs.

When I was with a carrier just last year, I had to cancel a 32 million dollar contract because my national coverage for DSL to grocery store chains was less than 50% nationally (and that was utilizing multiple providers).

Video is still a joke, and will continue to be that way until bandwidth comes down and user throughput is up. Businesses are struggling with distance learning and video conferencing because their employees can't get broadband out of their homes. How do I know? I sell it for a living.
