DAK, to the patients and their families the doctors who perform bypass surgery the value of the service is priceless but when that doctor goes to spend his income he cannot say I perform priceless life -saving operations please grant me unlimited credit. His spending & credit worthiness is limited by his nominal income & other assets of course. Assuming Bill Gates had a life threatening issue Would Bill Gates give 75% of his fortune to save his own life - I think so would he have to? no. His insurance would pay the going rate for whatever it was . mike
