Lunch time, so I need to be brief.

Even with the rise of superior medical care, there is not a great increase of people with Down's Syndrome or lacking apendages having further children of their own. There may be some, but mostly you see more handicapped because they are surviving to be seen, not mating and founding dynastys.

The type of variation that is on the increase is mostly one of sexual mixing. Genes that were perfectly useful but isolated in the world are now spreading through the population. These could convey resistance to a disease or pollution or a sense of beauty. A key gene in a time of selection will now be able assist many other genes in surviving because that gene will be present in a greater diversity of individuals and in novel combinations.

(Note to Kholt - evolution doesn't have a point, survival of the species is just the survival of collective individuals).
