The students are rarely their voluntarily; I am not satisfying the desires of the college students as customers, but laying down a series of healthy menus, whether they care or not; and my duty to the patient would be just the same if he were indigent or if I encountered him in an emergency, for example, if he had a heart attack and I called for an ambulance, and he protested entering it, and I cajoled him, using my position of authority to persuade him, rather than letting him die.
