How do you distinguished privileged from very privileged?

I would make the distinction based on whether or not we are prepared to write a check for all our education and medical needs or whether we are prepared to write the check only for whatever extras we want over and above what the voucher or insurance pays for. Many who may be thought of as privileged would be aghast, or impoverished, by the prospect of paying full price.
