If you are debating paying doctor's bills out-of-pocket without reimbursement you are very privileged indeed, whether you think so it an entirely different matter ;-).

I was debating it a couple of years ago, about the same time I was debating flying first class, about the same time the market was... Don't think it's really debatable any more. :(

Seriously, it's only doctor's bills. Hospital bills would be covered because all hospitals subscribe to Medicare. Those are the really big bills. So It's really not as much money as your first reaction might have suggested. Less than flying first class, for sure.
