I would have thought that it would be 100%. With an SEC registrant, an accounting firm is not permitted to qualify their opinion -- the SEC requires clean opinions.

Financial statements are the company's responsibility. Audits are not designed to detect fraud. If audits were designed to detect fraud, the cost would probably increase 5X.
