One reason for tying most medical expenses to private insurers is to show "what the market will bear", since people will only pay so much for insurance, and insurance companies will only pay so much for unusual procedures. That becomes the benchmark for any supplementary government health care, what is "usual and customary". One of the things wrong with the Clinton scheme was that it was ready to subvert consumer consensus as a benchmark, and put political fiat in its stead.......
