As for amplification within CDMA itself, I've been told by people I trust that 1X ev-do and ev-dv are pretty much the end of the line.

My whole point is that the end of the line is approximately the same for everyone and DO is very near it. The only way to get more is to do better compression or somehow isolate each cell from its neighbors (via beaming data straight from basestation to handset with minimal overlapping with other users - beam shaping and multiple antenna).
