That would be because TDMA systems are trivial. What, BPSK, QPSK, a bit of a guard band? This is in the domain of a tiny microcontroller, or a DSP.

Similar to AM radios, optimized for low-cost receivers at the cost of power efficiency, GSM is optimized for simplicity of the RF system at the expense of spectral-efficiency. Worked well, but in certain locations spectrum is no longer cheap or available.

CDMA systems require much tighter integration between various modules. A linear amplifier for multiple tones needs much more specification and design.
