I applied to Northern Illinois University in DeKalb, Illinois for my doctorate, I was accepted and taught two courses in general psychology while in graduate school there. But you don't say that you received your PhD there. Because you didn't, did you? You either dropped out or got kicked out. If not, state it bluntly: "I received a PhD from Northern Illinois University." Only you didn't, did you? You had to go to a diploma mill to get that.
