Like death doesn't really phase him - he is detached from it and it has been rendered an abstraction, fait accompli or as a fact of life. I couldn't possibly know his internal state but can judge from his emotional rendering that something is not quite right. I suspect that it is what soldiers develop when they have been in combat. Or suicide bombers.

How does one get this way? I think the environment shapes their brain chemicals. And eventually their brain morphology responds to the bath of chemicals. The two are connected and synaptic connections are now known to be growing and re-established throughout life.
