The most common cause of this error used to be the failure of the CMOS identification of the hard drive type, and this still sometimes happens. Another common thing today is to install a new device - say a CD - on the same channel as the hard drive and not set the jumpers correctly. Could also be a boot sector virus or other boot sector problem. If the hardware is OK it's probably "reload the OS" time... and hope you have a recent backup.
