I think we use it in many senses. So it means all kinds of people from chocolate fanatics (not dangerous, I think, unless you try to take away their Toblerone bars) to Wahhabi fanatics who are interested in violent revolution. All kinds of fanatics are out there. By using a sentence and many descriptive words along with the word fanatic, you can fully flesh out exactly what kind of fanatic you mean.
