There's an appraisal fee, processing fee, title insurance, escrow fee and several others. Maybe $2k total or more.

If you are keeping the home long term try to lock in 5.75% at zero points and pay closing costs only; you can usually recover closing costs in 24 months or less. If you have a time horizon of 5 years or less try a hybrid 5/1 rollover adjustable which gives you a fixed rate for the first 5 years (currently about 5.125%). Opt for a low margin over LIBOR in case you hold longer than 5 years.
