The answer may be if the stock in question is a pink sheet stock. If yes, they can and do issue discounted 504 paper which is immediately sold and not reflected in the OS.
A legal yet dirty scheme utilized by the likes of Nussen, et al.

If the stock is OTC, like ADOT (The number of issuer's shares of Common Stock outstanding as of June 30, 2002 was 168,435,223) volume can be misleading as double and triple prints occur and folks day trading for .0001 profit 10-30 times a day on 1 mil lots distort reality.
