I guess that through most of the history of civilization people must have died too early for their mostly grain diet to be a major problem. Earlier hunter gatherers probably got enough fat and protein but the masses in ancient and midevil times apparently ate mostly grain. The fact that they would be mostly whole grains might have reduced any potential problem with diabetes. Also the fact that they didn't have a lot of sugar in their diet probably helped.
