So far the only rational suggestion I read about how to avoid such dire straights, was a very lame "dont get in it in the first place". Of course, there's all kinds of opportunistic diseases (greed, double book keeping, derivatives...), which aggravate (considerably) the whole situation, but it's like Kaposi sarcome and AIDS. Seeing/fighting symptoms or going after the primary causes of distress.
