The problem with medicare is the artificial setting of prices which offloads cost on those not covered. This may have nothing to do with your idea. You could have a national pool and have insurers bid on chunks of it but there are some who will not be a part of whatever scheme we finally hatch because they don't believe in medical intervention or they would rather take the risk because they are young and in good health. Of course having a lot of healthy people in the pool to pay for the unhealthy keeps the average cost down but is it fair to them?
