In Okinawa, many of the defenders were teenagers, and there was a lot of collateral damage. But more than that, the Japanese had taken to deploying defense related installations widely throughout the population, using civilians as a deterrent for bombing. It would have been impossible to pacify the country without killing a large number of civilians, anyway. By the way, Truman considered Hiroshima to be a legitimate military target, because of this diffusion of war production among civilian neighborhoods. I do not know about the situation in Nagasaki......
