That is quite possible as they didn't seem worried about it. I wonder if we aren't going to find that the body was just old and over stressed. I hope that isn't the case because that is something they should be able to find IMO.

Sadly we find once again that space flight is anything buy routine.

Speed makes all the difference on whether or not something "relatively harmless" like the foam insulation on the launch fuel tank would be. At very high speeds, even "soft" materials can cause some damage, perhaps just enough to "tilt the scales" into the danger zone.
