Hawaii voted to join the U.S. as a state. Peurto Rico is still a territory and has not yet approved statehood.

Peurto Rico was annexed as a result of the Spanish/American war.

Hawaii was also taken in a military action.

The U.S. has such a big presence in Panama that we might as well have annexed it. Yes, I believe the Carter/Torrojos (sp?) treaty was finally implement under Clinton's watch and some Chinese concern took control of the canal. Although, I have to admit, I don't know that for a fact, it's just hearsay.
