The Brits were quoted as saying that Americans shoot at anything that moves. Was this biased reporting or fair?

Americans are taught at all levels to carefully select targets.

I can list a few things that happen in all armies that contribute to friendly fire casualties:

Weapons systems malfunction.

Friendly troop locations are not correctly reported.

Friendly troop locations are not correctly recorded.

Units or individuals are misidentified. Red/blue looks good on charts...but in the field nearly everyone (from both sides) wears green or tan.

Crew error.

Individual error.

Accidental discharge of a weapon.

Fear. Fear often causes soldiers in battle to fire a bit too soon.
