Friendly Fire incidents.

I doubt if there is much difference in the training between nations troops regarding fire safety. One factor that might have effected USA figures is the aggressive targets they set. This does not mean they are any worse, just they do more.

Apart from that, there maybe some disconnect between the military authorities in Britain and the USA versus "the best in class" regarding procedures and investigations.

I don't know, maybe the guys in the link below are just selling something that the military already does. It sounds to me if some as if they have a point though. From my own experience, getting out of "who's to blame" is the first step in any investigation. Sure it's important too, but usually only half the story (if that).
