I could be wrong (its happened once or twice!) but my impression from knowning many, many individuals in public and private life that those who have the courage of their convictions and know their subject are able to speak extemporaneously, and well. Liars and dissemblers eventually give themselves away, even with the best coaching. I've heard Junior Bush speak about taxes, without the usual hesitation or slightest mispeaking, leading me to believe he has both conviction and knowledge on that subject. Dean speaks well about everything else, on politics, defense, budgets and healthcare.
