It's true that the cost of obtaining new customers (advertising, commissions, and handset discounts) remained high, but subscriber revenues still covered nominal operating costs. As to prosecuting fraud, this requires a willingness on the part of local district attorneys, who were generally cooperative in pursuing a low grade felony at a relatively high local cost.
