There is a trend here that is ominous (to the extreme) for all fiduciary's in health care reimbursement systems: The Feds (Medicare), Self-insured companies (many, most of the S&P 500) and of course, HMO's:

CHICAGO, July 11 (Reuters) - Cigna Corp. said on Friday it cut its earnings estimates for the second quarter and year, citing steeper-than-expected medical costs and lower medical membership.

I guarantee, Cigna's main problem isn't lower membership...it's skyrocketing costs, with the lead sled dog being drug costs. It's a universal overarching trend, and it's accelerating.
