I believe that you will find that the power outage was caused not by inadequate equipment, but by hurried attempts to increase interconnects between grid subsections. This is being done rapidly to allow shifting of power sales across larger distances, basicly a repeat of the California problem without the highly public price gouging.

to install the improvements
The improvements need to be installed in Washington.

By definition it was some human error. To me by definition I don't need to hang anyone. The power grid is mind boggling complex machine. Something happened, find out what and figure out how to make it far less likely.

A faulty design in the program or equipment of load balancing.
A mis use of the equipment by accident or not understanding how it really worked.

It's summer, was a key experienced operator on vacation and the seconde string doing the monitoring.? Or maybe he was just in the crapper when the electrons hit the fan.

If in the automatic mode was an error made that did not account for the time delays in response to commands to shift load balancing.?

Did some equipment fail to operate correctly.?

Did some guy throw the wrong switch because of ignorance or panic?

In the spacial scope of hundreds of miles, electrons have mass and inertia. Just like one can see the waves in rubber banding trafic, current waves travel in the power grid. They get damped out is things work correctly. They get bigger and worse if thinks don't

In general the solution is to have more sources of power closer to the places that sink the power. In urban places the best solution is nuclear power mostly.
