yes there are users right now who are infected and they either don't have a clue that they are infected, or they suspect it and maybe even tried a few things to eradicate the problem, but they know they are not right and they are just "living with it"

there are many, many people in that boat right now

that's the key. the bad guys get in, patch things up with the latest ms security patch, clean up most of the infection, and leave themselves ingress and egress
