Accidents are preventable.

There are usually numerous reasons why an accident happens. A proper investigation usually reveals those many causes. There is very rarely a single person (especially just one guy at the bottom of the organization) who is entirely responsible. Going around checking all the other fuses is after the fact, what went wrong before the accident is also important stuff. Scapegoating one person (or one cause) rarely fixes the problem; in fact it hides all the other causes. I wonder how many real reasons why that wrong fuse was in place in the UK power cut? I am unimpressed with that announcement by the minister. In my estimation he needs to be fired just for saying that. I am not joking. There probably needs to lots of "one off" events like that amongst UK power management.

It happened and the "management system" failed to prevent it. The first step is to acknowledge responsibility for the problem. The "executive" is an idiot who doesn't have a clue imho.

The UK rail business needs some radical changes too. Starting at the top. Many saw most of the problems a long time before they caused an accident.

In fact UK big business organization seem to be in a downhill slide. I put it down to micro management, mobile phones, computers, and the "call centre" mentality, along with the brain dead view of the economy. -g-
