Kerry got his medals for turning a river gunboat defensive action, which was taking a lot of hit and run casualties (think Iraq now on the Mekong River) into an offensive action where they turned things around and went after the enemy. Kerry's bravery was legend, as whenever he got incoming from the shores, he'd charge instead of retreat, even beach the boat and charge into the trees. That made the enemy think twice about taking pot-shots. It worked.
Think APOCALYPSE NOW, same type gunboats.

He also killed a guy with an RPG who was about to blow up his boat (with his men in it) and he took three minor wounds.
