There was a time when "addiction" was associated through the popular conception with "derangement". It is unfortunate.

I am a firm believer in narcotics for pain control. Stocally enduring chronic pain is like refusing to wear a coat in a blizzard--only far far worse...

I have a simple advance directive with little detail. I also have someone holding my medical power of attorney who understands in his bones where I'm coming from. I supplemented that with some informal written musings about this and that, like pain medication, that he can use as a backup in case he is challenged. It seemed overkill to me to make sure every consideration was all formal. While it's not assured, I trust that my informally written wishes will be enough when coupled with the two legal documents. At the time I first did this, there was still a lot of pressure from the do-gooders not to honor such things, but we seem to be past that now.
