Randomization and groups baseline balance was compromised.

The article contains details of the randomization process. The randomization sequence was made before the study, and the patients were enrolled sequentially after they were qualified. Seems an impeccable process to me - but given the size of the study and the lack of stratification it's not surprising that the groups were somewhat unbalanced.

But the unbalanced groups matter a lot less than in most studies because this was not a cross-group study - here each patient was their own comparator.
