You are saying the ok came from the State Department without the knowledge and approval of the President. How are you coming to this conclusion?

From pieces of information picked up from an Atlantic article (I'd really have to go back and dig to find the ref now), and from knowing what State's standard modus operandi has been vis-a-vis wealthy Saudis for years and years. Remember, State was the department that would hand out visas to Saudis by mail, without even an interview - and it took over a year after 9/11 to get them to change the practice.

Why do you sound surprised about the President not knowing? Who would tell him? Don't you think he had enough on his plate just after 9/11?
