- parental custody This isn't a problem in Northern VA., which is an urban area, and I suspect that's true in most urban areas. May not be true in small towns.

- visitation right if the spouse in a hospital Simple, designate your lover as the person who has power of attorney in the event that you are incapacitated, via a health care power of attorney.

- maybe medicare and similar health care rights Yes, this is a problem, but they have their own medicare and health care rights. What they don't get are survivor's benefits, which are meant to go to non-working women. Social security was set up when women did not work outside the home, and still is set up that way.
