I think the real issue is that Dean may not know when to keep his mouth shut, which is different than someone who is a nutcase extremist. Dean just said whatever came to mind and it turned out to be something stupid. I can see him doing that in office too, btw and it is NOT a good thing. Otoh, Bush has the same disease and it resulted in some wonderful quips like "bring it on", "I'm sick and tired of <insert insult here to our allies>", "one bullet" and other non presidential behavior.

All I know is that after running as a moderate, and barely winning, Bush turned into some kind of extreme right wing neocon who for some reason thinks he has a mandate for all these controversial moves. He does not have this mandate and if the repubs think they can win by calling anybody (except maybe Sharpton or LaRouche) extremist, they are giving out ammo for the next debate, where Bush will be humiliated beyond repair.

I think Dean is a lot smarter than Bush and that will show in any confrontation.
