Interactive Brokers have good access to Canadian stocks.

IB has good accesss only on Toronto exchange stocks. No access to Vancouver exchange.

I buy Canadian stocks through Ameritrade, through their Canadian desk. $30 for unlimited shares, they place order through Canacord. Ameritrade is great because they offer margin accounts, you can trade options, and get free real-time quotes. A lot of Canadian juniors are now dual listed on the AMEX, more are being added all the time (see amex.com for a list). My experience is that I can get better fills at lower prices buying the U.S. listing rather than buying the same stock in Canada. You can buy these for $11 per trade through Ameritrade.
