I don't know if people would expect her to give up her career per se, but her desire to continue living the kind of life she lives now - practicing medicine and maintaining a high level of privacy, as shown in these articles on Dean's web page -(http://beta.deanforamerica.com/site/cg/index.html?type=page&pagename=about_JSD) would certainly clash with the high-profile life of a wife of a Presidential nominee or First Lady. She has no interest in politics, and you can't avoid politics in Washington. (Hilary had it easier in that respect - she gave up her law practice, but she had her own political career to think of.) I think most people would like her personally, she certainly has no political axe to grind like Hilary Clinton, and unlike the Clinton marriage what you see is apparently what you get, but IMO if Dean wins she will be very unhappy in Washington, even if she does practice medicine, because of the fishbowl existence.
