He is a simpleton. In Tom Dashiell's book, he describes a scene at a Presidential breakfast he has every week with Congressional leaders. The dress code is suit and tie. Dick Gephardt comes into the room with his jacket over his arm. Bush, standing by the door, tells Gephardt to put on his jacket. Gephardt looks at Bush, to share the joke, but slowly realizes the President isn't kidding. Blushing, he puts on his jacket.
