Why would anyone sell below cost for any length of time, except to kill off a competitor, which is inapplicable to this situation? A brief period to promote the product, maybe, otherwise, there would always be a profit.

The government is just another bulk buyer, in the case you mention.

The drug companies themselves can front- end development costs, and refuse to sell for less. The reason they don't is because it is not yet necessary.

Check on survival rates for various serious illnesses and medical procedures, for example. Check on the efficacy of care addressing chronic diseases (for example, the management of asthma or diabetes). Check on the link between infant mortality and the adherence to a disciplined prenatal regimen, including regular doctors visits. Check on the trend of earlier and more accurate diagnosis. There are all kinds of data that are useful in evaluating the increasing efficacy of medical treatment. If we are getting a bigger bang for the buck, what is there to complain about?
