Most bomb shelters had plans for second exits. Or at they were supposed to. The greater danger wasn't of a car landing on the hatch, but of the explosion somehow bending the metal so the hatch jammed shut. So as I recall you were supposed to have a place where you could knock a hole in the wall and dig a tunnel to freedom. After all, these shelters weren't buried very deeply. There was plenty of room in the shelter to store enough dirt to clear a path for a body to crawl out.
