I do the same - shop at Costco because it's closer, and also good quality. They tend to have the brands I want at a discount - the other day I saw they have my favorite brand of low-carb chocolate bars at a lot less than the health food store! Doesn't mean I'm going to stop going to the health food store, though.

I think the "Mom and Pop" stores will have to focus on special or unique items, and service. Also proximity - I like to search on the Web to see if any stores nearby have what I want, before I truck on out to the discount place or order it online. Unfortunately not too many local places have an online presence yet.

One criticism of Wal-Mart's health care plan is that it's great for catastrophic illness or accidents, not preventative care. However, there are reasonably priced alternatives (other than doctor visits) for things like vaccinations and flu shots. The emergency care clinics are also becoming popular - it's cheaper than a doctor visit and a lot faster than waiting in an emergency room.
