insane costs of some treatments (especially oncological)

With Erbitux and Avastin now leading the way. As a doctor friend of mine commented, Avastin is $50k for 10 months treatment and then you still die. Erbitux is even pricier I think (maybe 2x?).

Very hard to know what the answer is. And Medicare is going to be paying for a big chunk of it - my guess is that the newly raised cost estimate will still prove low.
