If the issue is seen a bit more broadly, i.e., a failure took place because the event occurred, and failures are unacceptable, then I can see somewhat of a justification for an apology.

There are cultures where that is appropriate. If a failure occurs, you resign or kill yourself, or some such. While you and I agree that apologies imply fault, we have so many subcultures and who knows what they think. I was involved in an extensive discussion on apologies on another thread. It was fascinating. There was a critical mass of opinion that one is supposed to apologize any time someone expects an apology based solely upon that expectation, the notion being that if someone expects it, it is warranted. I wish I could point you to the discussion. It was fascinating.
