No one knowingly gives unlawful orders. Those responsible are smart enough not to leave finger prints. You will not see them in photos.

General Karpinski was ostensibly "in Charge" but she was clearly not in control. There is a big difference.

In any large organization, there are always solid lines in the chain of command and there are dotted lines in the chain of command.

Sometimes the dotted lines take over.

It will be very difficult for the investigators to get to the bottom of this.

It is clear though that this is not an isolated occurence perpetrated by a few perverts. It appears to be systemic - but how high up it goes will be hard to track down.

But, as a people, we have to come out very strongly against this type of behavior and make sure that it can not be easy to do again.

It will be impossible to totally eliminate bad behavior. This is not to say that we can ever tolerate bad behavior. Worse, we can not tolerate policies that lead to this type of behavior. This can happen to any administration under any political system. But to excuse it in any way (jamal did it first, suhale did far worse, how come you didn't say anything when abdul dit it, yada, yada, yada) is almost as bad as committing the act ourselves.
