I think what Hersh uncovered is that this black program for interrogating detainees was meant only for "high value terrorist targets". But the methods and procedures that were approved for these guys was then transferred in a blanket policy to the detainees in Iraq, and specifically to Abu Ghraib. The CIA backed out last fall saying you can't use that method on cab drivers picked up in a sweep.

But strangely these methods went forward on the general population in the prison. The tactics were expanded beyond the high value targets. A classical Pandora's box situation.
