As far as how arbitrary the trimester system is I'll let the matter drop from now to avoid an "it is arbitrary", "no it isn't", "yes it is" conversation.

In most states, after the first trimester, abortion isn't allowed except in re. to medical emergencies.

In every state abortion is allowed past the first trimester for reasons of health that include "mental health" and other non emergency situations. The courts have made "health" such a wide loophole that it basically is the law not just a loophole.
