Of course they are separated the Persian Gulf, the Gulf of Oman and Iran, which itself has a culture quite removed from the Arab dominated Middle-East. Pakistan and Turkmenistan, which country extends much farther toward the mid-east than Afghanistan, are not considered Middle-Eastern and they are no more separated from the region than Afghanistan. The mention of Afghanistan in middle eastern studies departments does not occur because of any historical affiliation. It is a recent phenomenon, as recent as Taliban.

The fact is, it would not have made sense for Bush to refer to Afghanistan as an example of a Middle-Eastern democracy. had he done so, you reptilian numbskulls would have lambasted him.
