How much of the $332 is liability insurance ?

I had a crown which had popped off (chewing bubble gum on an airplane) , was put back on for about the same amount - a lot more work, and some materials. This in pricey Palo Alto....

California limits pain and suffering damages to $300,000, which keeps medical liability insurance down.
